Personal Care: Support with Daily Living Activities. We provide compassionate assistance with essential daily living tasks, including showering, dressing, and toileting. Our trained support workers are dedicated to ensuring your comfort and dignity while promoting independence. We appreciate that personal care is a delicate and personal aspect of daily life, and our team takes the time to build trusting relationships with clients, respecting their preferences and privacy. By tailoring our services to individual needs, we aim to enhance overall well-being and ensure a positive experience in daily routines. Community Access: Enabling Participation in Social Activities. We are dedicated to helping clients engage in their communities and participate in social activities that enrich their lives. Our support team assists with transportation ensuring individuals can connect with others and enjoy meaningful experiences. Domestic Assistance: Support with Household Tasks. We offer comprehensive support to help you manage household tasks, ensuring your home remains tidy and safe. Our dedicated team assists with activities such as cleaning, laundry, meal preparation, and general organisation, creating a comfortable living environment that promotes your independence. By taking care of these essential tasks, we empower you to focus on what matters most—your well-being and quality of life. Our approach is personalised, respecting your routines and preferences, and enabling you to maintain a sense of control in your home. Shopping and Food Preparation: Assisting you in maintaining a nutritious lifestyle. Our team helps with grocery shopping, ensuring you have access to healthy ingredients. We can also assist with food preparation, making it easier for you to enjoy balanced meals that cater to your dietary needs and preferences.
Medication Management: Ensuring medication adherence. Your safety is our priority, and provide our staff with additional medication training facilitated by a Registered Nurse, and we work closely with you and your healthcare providers to address any concerns or changes in your medication requirements.
Medical Appointments: Transportation and accompaniment to health appointments. Our team provides reliable transportation to ensure you arrive on time for your medical appointments. We also offer compassionate accompaniment, helping you navigate the process and providing support during consultations.
Enteral Feeding: Specialist feeding support. Our team is trained to provide expert assistance for clients with enteral feeding needs. Staff members undergo additional training facilitated by a Registered Nurse, ensuring they are equipped with the knowledge and skills to deliver safe and effective care.
Behaviour Support Services: We specialise in providing positive behaviour support to enhance the well-being and independence of our clients. Community Carers NSW is NDIS registered under Module 2A, allowing us to implement Behaviour Support Plans (BSPs) We are also equipped to manage restrictive practices when necessary, ensuring the safety and well-being of our clients while promoting positive outcomes.
Modified Disability Van: Enhancing Mobility and Independence. Our modified disability van is available for reservation to provide accessible transportation for individuals with mobility challenges. Equipped with features such as wheelchair ramps, the van ensures a safe and comfortable travel experience. Whether for medical appointments, community outings, or social events, our service is designed to empower clients by facilitating their participation in everyday activities. Our friendly and trained staff are on hand to assist with boarding and disembarking, ensuring a smooth journey that enhances independence and encourages engagement with the community.
Community Nursing Supports: Registered nursing care for complex needs. At Community Carers NSW, we provide specialised nursing care tailored to individuals with high needs. Our team of registered nurses is NDIS registered under the Community Nursing Care for High Needs (Registration Group 114), ensuring that we meet the highest standards of care. We offer comprehensive assessments and personalised care plans to address specific medical conditions, medication management, and ongoing health monitoring. Our compassionate nursing staff works collaboratively with clients and their families to ensure that all health and support needs are met, promoting overall well-being and independence in the comfort of home.
Meet Our Director, Annette Hanneman
With 28 years of experience in the disability sector, Annette leads our passionate team with a commitment to making a meaningful difference. Starting as a disability support worker, she pursued her Diploma of Social Work while gaining hands-on experience in various roles, including managing group homes and early childhood intervention programs. Her personal journey caring for her father, who suffered an acquired brain injury and later dementia, fuels her dedication to providing high-quality care.
